Content
Content of the course: The purpose of accident investigations. - Investigation as part of a systematic safety management system. - Accident theories and models - Examination and learning of events in practice. - Investigation on three levels - Planning of the investigation and investigation process: mandate, team and organization, preparedness and notification, on-site investigation, interviews of victims and witnesses, analysis and what, how and why it happened, findings, recommendations and reporting - Methods of data collection - Methods of accident analysis - Learning after incidents
Learning outcomes
Knowledge: After completing the course the student shall be able to:
- explain the purpose of accident investigation
- explain principles for systematic investigation of accidents
- explain the chain of events using accident models
- choose the right type of investigation level and methodology for different types of events
Skills: After completing the course the student shall be able to:
- use a framework for efficient safety management and learning to improve accident prevention in an organization
- collect data about incidents
- systematize and model the chain of events
- analyze direct causes and underlying causes of events
- propose and argue risk mitigation measures
- analyze the results of investigations
Competence: After completing the course the student shall be able to:
- plan and lead an independent investigation committee
Teaching methods
Classroom teaching and discussions. Conduct the steps of an accident investigation on a case.
